P4 form questions

I downloaded some copies of the forms in the P4 packet. I figured it would be easier for Sarah. I did the same with the P3 forms and it proved to be a good choice.

 

QUESTION

On the forms, it asks for "Alien (Case) Number". Should the GUZ# be inserted there?

The only place I found this question on my p4 forms was on the medical forms. To be honest, I left those forms blank and told my wife to hold-off until she saw the doctor and then the doctor would tell her how to fill it out (which is generally the case). When your wife/fiancee goes for her medical appointment, have her take her other forms, filled out from her p3 and p4. There is someone there who should be able to explain who to fill everything out on the medical forms.
