Notarial Birth Certificate

Hello!

 

My wife and I are applying for green cards (I-485) and we need to submit a copy of her birth certificate.

We have a one-page document that looks exactly like the sample document below, except:

The title at the top is Gong Zheng Shu instead of Chu Sheng Gong Zheng Shu

It doesn't have a number (like XO01836888) in the lower-left corner

It has her information and photo, not somebody else's information and photo

 

Is this sufficient for I-485 (along with a professional translation), or do we need to get a 4-page white booklet? How would we get the white booklet, given that we aren't in China and don't have family or friends in China who could take care of it for us?

The serial numbers tend to show the document and related attached documents are related, the numbers tend to be sequential.

 

As for USCIS requirements all they need is a photocopy of what you have and a translation. Note translation does NOT need to be professionally done, anyone who is conversant in Chinese and English can do the translation, they then make notation at the bottom of the translation that they are conversant in both languages, certify that it is a true and accurate translation, they sign their translation, date it and provide contact info, just in case USCIS wishes to call them about it.

What you have is simply a Notarial Statement (Gong Zheng Shu). The white book format is the Chinese standard for birth certificates for use overseas. The document you have may or may not be accepted. But the only alternative would be for someone in China to get it for you.
