About notarization for used name

This is a common question. A lot of people asked if they need an "used name notarization" when they filled their used name in the used name blank on all the forms .

 

Because in china, a lot of Notary office don't write used name on the birth notarization, so the question is if their birth notarization didn't show their used name, should they go to make an used name notarization separately in case?
