Medical forms both English and Chinese

My SO has more questions than a 6 year old child

and this one is a zinger !!

Since the medical forms have to be filled out in Both English and Chinese

what do we use for My name on the Chinese Form

should it be my English name , address , phone number etc

or should there be some translation into Chinese ?

I don't see how they can translate my name etc into chinese but the So says that it could be done Phonically

The English name should be fine and dandy. It's not easy to translate English name to Chinese. There are two ways of translation - 1. by phonetics, or 2. by the meaning of the word.
